How is PMDD diagnosed? The DRSP explained
There is no blood test for PMDD. Reliable diagnosis requires prospective daily symptom ratings over at least two menstrual cycles. The DRSP is the internationally recognised gold-standard tool.
Why is there no blood test for PMDD?
Hormone levels are usually normal in people with PMDD. The difference lies in the brain’s sensitivity to normal hormonal fluctuations, which one blood sample cannot measure. Blood tests can still help rule out other causes, such as thyroid disorders.
What is the DRSP?
DRSP means Daily Record of Severity of Problems. Developed for clinical research and used internationally, it aligns with DSM-5 criteria. You rate 21 symptoms every day from 1, not at all, to 6, extreme, plus three yes/no interference items covering work or education, relationships and activities. See all 21 symptoms.

How does the diagnostic process work?
- Rate all 21 symptoms every day, ideally at a regular time. This takes around two minutes.
- Complete at least two full cycles to establish whether the pattern repeats.
- Compare phases: symptoms should be clearly higher in the luteal phase and minimal or absent in the follicular phase.
- View the cyclic overview: based on the C-PASS method, it makes patterns visible without diagnosing.
- Discuss the results with a clinician, using your ratings or PDF report.
- Consider other diagnoses, including thyroid disease, bipolar disorder, depression, anxiety and physical causes.
Which clinician should you see?
A GP or primary-care clinician is usually the first point of contact and may refer you to a gynaecologist or psychiatrist. Say clearly that you suspect PMDD and bring your prospective ratings. A psychologist may also support treatment.
